SHINE Community Services is jointly funded by the Towns of Mosman Park, Claremont and Cottesloe and the Shire of Peppermint Grove along with the State Government’s HACC programme.
Their services cater for seniors or younger people with disabilities, and carers of both groups. Persons eligible for the programme must reside in the local government areas of Claremont, Cottesloe, Peppermint Grove and Mosman Park. HACC eligible residents can reside elsewhere.
Services are designed to help people to continue to live in their own homes. SHINE Community Services' aim is to promote independence by helping clients find new ways of performing tasks and by only carrying out tasks which people are physically unable to perform themselves, as well as promoting well-being and an active social life.
The first time a client contacts SHINE Community Services, details are required, such as name, address, phone number, Doctor and a brief medical history of the client. When receiving a subsidised service, each client will be asked to complete an Income Assessment Form, simply specifying where they are on level 1 or level 2 income.
