West Australians who are at risk of, or experiencing homelessness, are some of the most vulnerable in the community and require support to end homelessness. Tackling homelessness requires a strong focus on intervening early to prevent homelessness, and ensuring a range of flexible and responsive services are available to break the homelessness cycle. It is recognised that the response to homelessness also requires an integrated service system from all sectors of the community.

Street to Home program

The State Government through the Department for Child Protection and Family Support has established the Street to Home program. This program is an interagency, integrated response to the accommodation, general health, and other support needs of people sleeping rough. People sleeping rough are assisted to access stable accommodation, and address issues surrounding their homelessness to achieve positive, long term outcomes.
Outreach Team’s work to engage with individuals who are not accessing traditional homeless services and aims to build trusting relationships in which support can be provided to address a range of life issues, improving quality of life and accommodation options. The Assertive Outreach Team’s work alongside the Mental Clinical Outreach Team and liaises with Supportive Housing workers to access and maintain suitable accommodation.
